PLEASE READ the following prior to registering.
Each family must RSVP for this event.
You will be assigned a time slot to arrive to Children's Lantern for book bags, supplies, and shoes.
Once finished at Children's Lantern, your family will then go to Cut and Polished Salon in Defiance for free haircuts (if you so choose). If your family would rather receive haircut vouchers to be used at another time, please specify below.
We must operate on a timely schedule for this event to run smoothly and to coordinate with the stylists volunteering their time to give free haircuts. Please make sure you fill this form out in its entirety so we can accommodate accordingly!
**This event is open to any foster, adoptive, or kinship family in Northwest Ohio and is for every child within that family whether bio, adopted, foster, kinship, etc.**

The flow of events will be:
1) Arriving to Children's Lantern at your scheduled 30 minute time slot.
2) Once finished at CL, the salon will expect your arrival for haircuts immediately after.
No family should arrive at the salon until after they have already been to Children's Lantern.
The Salon will have a copy of each family's expected arrival times.

Please format the information as follows for EVERY child who will receive supplies & shoes. This will ensure we have the correct school supply kit for each registered child. EXAMPLE. L.P, 1st grade, little girl 13 shoes G.P, 4th grade, womens 6.5 shoes D.P, 9th grade, womens 9 shoe.
Please note: Once your form is received, we will review it and then email a confirmation with arrival TIME back to you within 7-10 days. If for any reason you are not able to attend or if you have time conflicts, please email us so we can accommodate changes to the best of our ability.
